Obstructive Azoospermia

In this condition, sperm is produced in the testes but can not be found in the semen. It happens because the male reproductive tract gets blocked. Though, sperm can still be retrieved through surgical sperm retrieval (SSR) procedures such as TESA to be paired using IVF.

Some of the reasons that can cause obstructive azoospermia are: sexually transmitted diseases (STIs), scar tissues due to injuries, genetic conditions, and previous surgeries.

Non-obstructive Azoospermia

In non-obstructive azoospermia, mature sperm is not found in the semen. It happens because of reduced sperm production. The pituitary gland secretes a hormone that stimulates sperm production. The absence or inadequate amount of this hormone affects sperm production.

Genetics, Y chromosome deletion, karyotype abnormality, radiation and toxins, medication, hormone imbalance and varicocele are some of the reasons responsible for non-obstructive azoospermia.

Retrograde Ejaculation

In retrograde ejaculation, semen enters the bladder instead of coming out of the body through the penis. There is very little ejaculation or no ejaculation. Cloudy urine because of the semen present is a typical symptom.

Retrograde ejaculation can be a result of diabetes, medication (especially drugs used to treat high blood pressure), injury or past surgeries.

During treatment, sperm can be retrieved from a urine sample and used during an IVF/ICSI cycle. A surgical sperm retrieval procedure can also be used to retrieve sperm for IVF.

Oligospermia

In oligospermia, the semen has a lower concentration of sperm than normal. The concentration of sperm is considered lower when is below 20 million sperm/ml. Very often a low concentration also means decreased sperm morphology or motility.

Various reasons such as poor health, ageing, testicular trauma, chemotherapy or radiation may be responsible for oligospermia.

Depending upon the cause, the diagnosis begins with a supplemental hormone regime to improve sperm quality, followed by IVF.

Varicocele

Varicocele is an enlargement of the veins within the scrotum (skin bag that contains testicles). Blood accumulated in the veins rather than circulating out of the scrotum causes varicocele.

Varicocele may hinder the normal development of a testicle and can cause low sperm production or other problem that may lead to infertility. It can be treated by micro varicocelectomy.

Varicocele may cause discomfort or pain, but often has benign complications. It is usually harmless.
